What is gastric bypass surgery? How is gastric bypass surgery performed?

Gastric sleeve gastrectomy is the most preferred bariatric surgery method in the world, followed by gastric bypass surgery. With gastric bypass surgery, both a small pouch (gastric pouch) is created above the stomach and the part of the small intestine 50 cm below is connected to this pouch. In this way, no food passes through the stomach and the first 60 cm of the small intestine. The most important difference between gastric bypass and sleeve gastrectomy is that in gastric bypass surgery, food does not pass naturally. After gastric sleeve gastrectomy, the food passes to the stomach outlet and from there to the small intestine, that is, it is absorbed in its natural course. In bypass surgery, the food that does not pass through the gastric outlet bypasses the first 60 cm of the small intestine. Since the absorption of drugs and vitamins in these first 60 cm is very important, vitamin support may be required for many years after gastric bypass surgery.

How is sleeve gastrectomy performed?

Gastric sleeve gastrectomy is performed closed (laparoscopically) by entering through small holes in the abdominal wall. Since the abdominal wall is not cut in sleeve gastrectomy, the person can stand up and walk on their own after 1-2 hours. The large part of the stomach on the left is separated from the surrounding tissues, then the anesthesiologist advances a silicone 12 mm diameter tube through the mouth into the stomach. Under the guidance of this silicone tube, the stomach is cut on the vertical axis using a stapler (stapler) tool and the stomach is tubularized in the shape of a banana following the esophagus. The large part of the stomach (about 70%) is removed through the hole in the abdominal wall. The stapler line is reinforced with a second suture line before the end of the gastric sleeve surgery.
